Transitions of Care Summit

Recorded April 2018 at Anticoagulation Forum Boot Camp in Austin, TX  this program outlines why anticoagulation patients are at increased risk when changing care settings, complexities in care, and best practices for discharge. 

Learning objectives: 

  • Demonstrate awareness of complexities in transitions of care including bridging & peri-procedural care, referrals & transitions between patient care settings
  • Use methods and techniques for safe transitions of care for VTE patients treated in an ED, to home or hospital
  • Outline best practices for safe discharge of patients on anticoagulants
